“Desire starts in the brain,” the drug’s website reads. The problem is the media’s insistence on comparing Addyi to another, more well-known pastel-colored pill. “It’s known as Viagra for women,” a newscaster’s voice announces, barely a minute into The Pink Pill: Sex, Drugs & Who Has Control, a documentary that premiered at DOC NYC earlier this month and charts Addyi’s fraught, tortuous journey to FDA approval. Now, women have the pink pill, Addyi, the first FDA-approved treatment for low female sexual desire. As the film shows, the legacy of Viagra has plagued Addyi from the start.

Although Addyi doesn’t work on the genitals, it was reviewed by the FDA’s reproductive and urological drugs division, which also approved Viagra. The FDA required Addyi to show in clinical trials that it increased the number of “satisfying sexual events” a woman had per month, mirroring the requirement that Viagra boost a man’s number of “successful sexual events.” (Successful, in this case, meant that penis was rigid enough to penetrate vagina.) Think of it like this: When you crave a birthday cake—imagining the funfetti frosting melting on your tongue—that’s desire. When your mouth waters and your stomach gurgles in response, that’s arousal. Aisling Chin-Yee, the Canadian filmmaker behind the documentary, is aware of the distinction. “It is super cringey to call it the female Viagra for so many reasons,” she told me. (More reasons: Addyi has to be taken every night, not just when one might want to get busy, can’t be combined with alcohol, and might make you faint.) Starting the film with the comparison, she says, gives viewers a launching-off point to consider the double standards at play in the fight to get Addyi into doctors’ offices and women’s medicine cabinets. But while the film does explain Addyi’s mechanism of action, framing a female desire drug as the equivalent of a male arousal drug echoes ancient assumptions about the nature of male and female sexuality. The implication is that for men, sexual problems are all in your penis.

For example, doctors and pharmacists had to take a test, known as a Risk Evaluation and Mitigation Strategy assessment, to prescribe and administer Addyi. The assessment included an online slide presentation followed by a comprehension test that proved the medical professionals knew and understood the drug’s risks of potential low blood pressure and fainting when combined with alcohol. Even further, patients had to sign a paper vowing not to drink while on the medication even though Cialis--another medication used to treat erectile dysfunction in men--creates similar interactions with alcohol but comes with no more than an advisement to moderate alcohol use. At the time of approval, the FDA argued that Addyi’s interaction with alcohol hadn’t yet been adequately studied. In 2019 and after additional research was conducted, the FDA modified its original alcohol warning for Addyi and removed the test requirement for doctors and pharmacists.

In December of 2025, the FDA approved the drug for postmenopausal women, expanding access to more patients. Advocates say a drug to treat women’s sexual dysfunction is a start to addressing health inequity. Lee adds that recent bigger picture events, though, like the overturn of Roe v. Wade and rollbacks on birth control access, have changed what health care looks like for women today. The approval and acceptance of Addyi is part of a much larger and ongoing conversation about women’s health and regulatory policy currently.

“The past five to 10 years really calls into question where the state of women’s health is going,” Lee said.
